ATTENTION GRADUATING STUDENTS!

FORMS REQUIRED (or optional) WITH YOUR GRADUATION APPLICATION!

 (All forms-- there are 6 --can be accessed online at this site or University site)

1.    Application for Graduation (cover sheet with signatures of student and advisor)

2.    Student Progress Report form (can obtain online with account name and pin.  According to Mary Brown from the Registrar’s Office, this form is very similar to each student’s Student Study Plan.  Everything that appears on their Student Study Plan should also appear on the Student Progress Report.)

3.    Student Study Plan-This usually is filled out your first trimester with your advisor’s assistance and signed by him/her.  PLEASE SEE YOUR ADVISOR IF YOU HAVE NOT COMPLETED THIS FORM!

4.    Exception Form (UG only) is to be used when there is a course substitution  or a student is undeclared and wants to take a course. 

5. Transfer of credit form (Graduate Students Only) Required if you want to transfer classes from another college or university for credit toward you GSU degree-- 25% of degree program maximum and the transcript must accompany the form.

6. Student Progress Report Change Verification form is to be used when the Graduation Applications have been returned to the advisors by the Registrar’s office and the student has changed his/her mind about the courses that he/she agreed to take.  For example, if a student signs up for a course and the graduation application has been reviewed and signed off by the advisor, then the student decides to drop one of the courses in the present trimester without consulting with his/her advisor first, and decides to replace that particular course with another course, this form would be used.
